Insurance is not accepted at this time, however, some insurance companies allow for possible reimbursement via submitted receipt or your physician's recommendation for nutritional services (please request this form from us).

Initial Interview, Nutritional Assessment Questionnaire, Food & Reaction Journal.2 Please fill out these forms as thoroughly and honestly as possible. Happiness is Health is a judgement free zone. If you eat donuts everyday, smoke on Saturdays, only eat bananas, watch 7 hours of television a night, or jog 10 miles a day, please include these, as this information is an important part of your health.
5. SCHEDULE APPOINTMENT
We will discuss your information, have a Q & A, interpret your nutritional status, and discuss recommended protocols. Future appointments will be scheduled as needed or per chosen program.

1. Services will not be rendered until Disclaimer has been signed and returned.
2. If you would prefer paper versions of these forms, please let Christine know during the initial consultation. The forms will be mailed via USPS after you have prepaid for services using the above links and submitted your Disclaimer. Please return all forms in the self-addressed, stamped envelope provided. If you require a new, blank form, please use request form below.
My Polices:
Due to the fact that we cannot guarantee that a client has followed recommended protocols and/or dietary and lifestyle changes, refunds for services are not issued.
If a client has entered into an agreement/contract with Happiness is Health, and has prepaid for services, but was unable to continue due to proof of a medical emergency within 14 days of payment date, a full refund will be issued within 60 days, per client/guardian written request.
My policy allows for 1 reschedule at no charge after missing an (original) Nutritional Assessment, Functional Evaluation, Root Cause, or Follow Up appointment. If a rescheduled appointment is missed, there will be a charge of $50 to reschedule, or the appointment will be forfeited with no refunds. All original and/or rescheduled appointments must be within 90 days of originally submitted intake forms, or client's Nutritional Assessment will become Null & Void, due to the fact that nutritional status can change after this amount of time and must be reassessed utilizing updated intake forms, updated agreement, and additional payment (if appropriate). Since the Nutritional Assessment portion of the agreement had been completed by Happiness is Health,
no refunds will be issued for client non-participation.
