Please fill out these forms as thoroughly and honestly as possible. Happiness is Health is a judgement free zone. If you eat donuts everyday, smoke on Saturdays, only eat bananas, watch 7 hours of television a night, or jog 10 miles a day, please include these, as this information is an important part of your health.
My Polices:
If a client has entered into an agreement with Happiness is Health, and has prepaid for services, but was unable to start due to a medical or accidental emergency before beginning services, a full refund will be issued per client or guardian written request and proof of issue. If a client has already begun nutrition services and a medical or accidental emergency occurs, a pause in their program can be discussed. Please be sure you are ready for Nutritional Therapy before signing up, ie. you have the time, finances, and are mentally ready to make changes.
Some insurance companies allow for reimbursement via Nutritional Therapy receipt or your physician's recommendation for nutritional services (this form is in every initial packet). Insurance is not accepted at this time, however, may be offered in the future, please inquire.
Notice is required for rescheduling or cancelling an appointment.* If an appointment is canceled, rescheduled or missed without sufficient notice, a fee of 25% may be charged to the credit card on file or invoiced to client.
12 hour notice required for Telehealth appointments.
4 day notice required for In-Person appointments.
*Please let me know if cancellation was due to an emergency.
