1. DISCOVERY CONSULTATION (15 minutes)
2. HEALTH INTAKE FORMS (45 minutes)
3. INITIAL ASSESSMENT APPOINTMENT (60-90 minutes)
4. FOLLOW UP APPOINTMENTS (30-60 minutes each)
5. FUNCTIONAL & LAB TESTING (as needed basis)

Please fill out Intake forms as thoroughly and honestly as possible. Happiness is Health is a judgement free zone. If you eat donuts everyday, smoke on Saturdays, only eat bananas, watch 7 hours of television a night, or jog 10 miles a day, please include these, as this information is an important part of your health.
Functional & Lab Testing:
Bloodwork, Food Allergy, GI Map, NutriStat, Organic Acids Test (OATs), Applied Kinesiology, Muscle, and Lingual Neuro Testing (LNT)
My Polices:
Please be sure you are ready for Nutritional Therapy before signing up... you have the mindset, time, finances, and are ready to make changes. No refunds issued.
Insurance:
I do not accept insurance at this time. Some insurance companies allow for reimbursement via Nutritional Therapy receipt or your physician's recommendation for nutritional services (please ask for this form). HSA and FSA can often be used to pay for labs, please inquire with lab before scheduling your appointment.
Appointments:
If an appointment is canceled, rescheduled, or missed without sufficient notice (see below), a fee of 20% will be charged to the credit card on file or invoiced to client. If you are more than 10 minutes late for your appointment, you will be responsible for payment of original appointment time, or asked to reschedule and charged a fee.
12 Hour notice required for Telehealth.
4 Day notice required for In-Person.
